Health Technician (Patient Navigator)
This position is located in the Emergency Medicine Department of the Gallup Indian Medical Center. Incumbent serves patients through a variety of duties associated with screening/referral of patients to treatment resources as well as related duties and responsibilities related to patients diagnosed with: substance use disorders, HIV, viral hepatitis, sexually transmitted infections and similar issues. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Health Technician Total Compensation | Pay (ihs.gov) provide referral support services in the clinic, community, and home-based patient care, through the identification of services such as: immunizations, testing, referral for medical therapy services, support, and /or housing needs under the Director of the Emergency Department. Identifies and coordinates access to a variety of additional services, such as mental health services, primary care, social services, and outpatient and residential treatment programs. Introduces patients to appropriate programs and services, facilitating appointments at outpatient clinics and follows up on referrals to help ensure patient is utilizing resources. Assists with patient navigation services needed for patients admitted to inpatient units in the hospital as well as follow-up with patients after discharge from another facility if transferred after release from jail, incarceration, or a substance use rehabilitation program. Participates as part of a health care team for their clients assisting with issues such as substance abuse to help provide services and mentorship. Provides on-site visits, as necessary, to in-patient facilities and community locations as part of a health care team. Provides interpretation to health care providers the health problems, needs, and responses of patients and concepts of culturally specific values, beliefs, and practices. Obtains pertinent information from patient, tribal representatives and/or tribal organizations as well as resource organizations to determine and support culturally sensitive and appropriate resources while taking into consideration cultural values, beliefs, and practices to ensure resources meet the needs of the patient and support successful outcomes. Maintains all patient information, which includes lab values, demographic data, and appointment time for clinic activities and home visits; maintains database using appropriate of patient encounters and reason for encounter.
Requirements
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Specialized Experience (for positions at GS-4 and above): Positions in this series range widely in type and include support duties to medical or health personnel such as audiologists, speech pathologists, medical officers, and optometrists. Therefore, technician experience is experience that required application of the knowledge, methods, and techniques of the position to be filled. OR Education and Training For GS-5: Successful completion of a full 4-year course of study leading to a bachelor's degree, with major study or at least 24 semester hours in subjects directly related to the position. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Experience assisting public health professionals in providing public health services to individuals, families and communities; conduct home visits; implement care plans; provide health education to individuals, families or groups; prepare statistical reports. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
